Forms of Transaction

In order to maintain a clear record of use of items and donations, the Shepherd University Archives and Special Collections (SUASC) uses the documents listed in the boxes below.

Item Request Form

Materials in the SUASC are requested using these forms; the slips are given to the archive manager to retrieve. Use of materials occurs in the Archive Reading room.

Archive Reading Room Policy

Researchers must read, agree to, and sign one of these forms every academic year (July-June).

Deed of Gift form

 A Deed of Gift transfers all rights of ownership of the given materials to SUASC.
